CHANGES TO OFFICE VISITS DURING COVID-19
- Upon entering our suite, we will have readily available hand sanitizer for your use. Also, when entering our suite we ask that you please wear a mask. Any person accompanying the patient must also wear a mask.
- Upon entering our suite, we will take the patient’s and any accompanying person temperature. If the temperature reading is greater than 100.4, you’re feeling sick or you have a sick family member at home, your appointment will be cancelled.
- Upon entering our suite, you will answer the supplemental informed consent/questionnaire form that can be downloaded from your phone scanning the QR code at the front desk.
